If your student has any medical issues such as asthma, seizures, food/environmental allergies, etc., please print the corresponding forms (below) and have them completed by your physician. Forms must be turned in at registration.

If medications will be required to be given at school, please have your physician complete the medication form. Parents will be responsible for bringing the medication with the completed form to the school to be turned into the nurse.
